Hi there! I am Danni, an experienced healthcare worker, Florida A & M University Masters of Social Work graduate, and author of “Totally Organized, Even Though I’m Not Here: End of Life Planner”. I have over 12 years of experience within the healthcare field. During my work experience as a Hospital Medical Case Manager, I have noticed the need for Social Work Case Managers post-discharge and to prevent any hospital readmissions. Social Work Case Managers, also known as the Personal Service Coordinator, provide support and guidance that empowers individuals and families to take a more active role in managing their own care and advocating for their needs, building their confidence in the process. Utilizing a faith-based approach and social work knowledge and skills, I enjoy connecting individuals and families to community resources and coordinating services to keep clients well. My duties include knowing all of the pieces to the puzzle and helping to place them together. Though often this is a role that falls to a family member, it is a role that can become overwhelming for family members to take on. I aim to bridge the gap between the healthcare world and clients/families while providing personal support. I am grateful for the opportunity to continue serving others while spending more time with my family.
